Don't know what to put on my occupational health form!

I am filling in a form for a job I am applying for and it is asking how many days sick I have had in the last 12 months.

I have been off quite a few times but I can't remember the exact amount, also even if I put 6 or 7 however many it is, will this count against me?

It may do it may not - it depends on the employer really but it is best to be honest.
