Hi ya......are you hyperventilating, or could you possibly have indigestion? or maybe done some physical activity and strained a muscle?

I think it is highly unlikly for you to be having heart attack, you would have other symptoms along with the shoulder pain. I have experienced chest pain and shoulder pains a number of times and its always down to something like tapped nerve/strained muscle or the fact that I am never breathing right which means aches and pains popping up around that area! Hope this helps a little.

It sounds tension related to me. Whilst it may be muscular, nuscles have memory and will continually re knot when we are tense.

A deep massage may help.
