Not sure if I can promote this but if I can I would tell everyone to read this book! Matt Haig, the author, suffered from depression and anxiety and tells all in this book. I read it in a day and since then I have had a new outlook on my anxiety! If I am allowed to suggest it or recommend it then I would urge everyone to read it!

After reading your post I spotted this at a friend's, borrowed it and read it yesterday. I found it had several really useful parts (the positive lists) but I felt it didn't really explain exactly how he got better, which is the part I was most interested in.
It has a lot of detail about his breakdown, which might be worth knowing for new readers as this could be a trigger.
