I have a small shiatsu massager and i have been using it on my back because of the left front and back ache i have been having for a few months. The massager hurts on my left. I have been freaking out that it's an organ so if it was would the massager not hurt? (if that makes sense)

Because your pain is muscular, it will hurt in that area until the massage starts to release the tension in those muscles. Just give it time to work.

Just worried it was an organ and not muscular because of how long it's going on.

massager aside, does it hurt more in certain positions. like if you twist or bend over?

It hurts if I twist my torso and if I stretch/bend over.

Needs to be an acute trauma to recieve pain from an organ.

it's muscular
pain from an organ wouldn't change with movement, it would just persist at the same intensity,
your answer clearly indicates a mechanical pain. usually caused by bad posture .I would leave the massager alone , it's not medically trained and you don't need a relaxing rub.
you need a few deep tissue massages or sports injury
I'm not gonna lie , they hurt . but they work.
