I have had a left back ache that radiates into my shoulder blade and shoulder for about 3 months. If it was my lung would it hurt if i laid on my back etc?

It would hurt more when you inhaled, but even that can be caused by a tense muscle as the muscles expand and contract when you inhale and exhale.

You've clearly got a pulled muscle or something of that nature.
