Can a CBT Therapist Provide a Doctor's Note & Diagnose?

can a CBT therapist diagnose people with Agoraphbia for example? Doe sit then go on your medical records? Can they provide a doctors note as a GP would?

Hi I don't think they can have you talked with your therapist about getting help for your Agoraphobia it is treatable :) ATB
