Eyes keep going out of focus

This happens a lot, but it seems to be mainly when I’m stressed. I have to shake my head to regain normal and clear sight again. It’s like I’m zoned out but with double vision until I manage to snap my self out of it. Is it anxiety?

I get this all the time and it's normally when I'm really tired which again is all the time 😂 Is it like you feel like you're in a trance and can't look away? '
