anxiety about leaving work/starting uni

hey there, i work at a cinema however i want to hand in my two week notice at the weekend as i am starting university the beginning of sept and i want some time off to prepare for uni and sort everything out. i've been in the same job for over two years and i'm a bit worried about handing in my notice. i have no idea what to write. i was just wondering if someone could do me an example letter for me to copy? it would really help me.
