Sharon - google "cbt thought error analysis" and there are various websites and forums which explain what to do. There are also a couple of old threads on here about the subject. You'll find them if you use "obitz' in the search thingy x

TEA forms stand for Thought-Error-Analysis. You just need a piece of paper and pen to do them. So basic but they work great. They are outlined in the short CBT book by Sam Obitz called "Been There, Done That? DO This!" I still use them as part of my almost daily routine to keep my anxiety away:D
