Too afraid to up my dose

I've been given 40mg instead of 20mg, but I'm a little nervous about going up a dose because of the side affects.

Hi, my advice to someone who is afraid to take their meds is always the same:

Get it down you!

I'd recommend going up to 30mg first, then if you need more after 1-2 weeks you can go up to the full 40mg. If you are taking 20mg tablets, simply cut one of the tablets in half. If you've already adapted to the medicine's side effects, you are in a very strong position to cope with an increase in the dose.

Also there is no benefit whatsoever on staying at a dose that is too low. Don't be afraid, just get it taken. The sooner you start, the sooner you will start to notice the benefits. Go for it!
