    CBT refused !

    Hi,
    I'm in a bit of a state.
    Have been to 3 screening assessment sessions for CBT.
    I told my Senior Nurse Practitioner about some crazy episodes 24 yrs ago
    that got me sectioned, imprisoned, thrown out of a therapy house.

    1990 was my annus horriblis, couldn't get it together.

    Now all she wants to do is home in on those episodes.
    saying that "triggers" might make me react in the group CBT.

    I haven't had any trouble with Police in 24 yrs.. and
    all she's doing is dragging me over the coals about the past.

    Says she needs to be sure I'll be safe in the group.
    fair enough , to a degree, however its messed up people
    like me that need the help, why is she setting the bar so high?

    sectioned: I pulled a knife on someone, didn't use it - cry for help:
    imprisoned : I hit a shop seller outside a 24/7 , ( my pal had nicked a scotch egg )
    thrown out hostel: slapped someone lightly across the face, more in jest.

    That's it ...
    since then I've had time to understand my behaviours, become sensitive.
    Why is she breaking my balls? winding me up?
    I am keen to do the CBT, commit to it, and believe she's over-reacting.
    Not allowed to join a group - the inference being I'm a danger.
    I find it really sad and irritating she is taking this attitude.

    Treatment denied ! I'm not worthy? good enough ?
    which is exactly some of the thoughts i have which get me down.

    Meds + forums + self-help = good -- NHS no help = bad

    Do forth to hear that. It's silly considering you need help!! Could you ask to be referred to CBT over the phone. Therefore you're not in a group scenario. I wax offered CBT over the phone but asked for 1:1 don't know if I'll get it.

    If not can you go to a different doctors or write everything you said in a letter to the surgery manager? Is their duty to offer you some help.

    My GP is fine with it,
    it is rather the NHS Nurse who does the assessments.
    My psychiatrist met with her and myself today.
    I was offered more meetings for "an assessment for CBT" and said no.
    It'd been a waste of time.
    I've asked them to write to me.
    Not hopeful at all , even though the shrink recommends it.
    A difference of opinion.
    Its disappointing and has put me down in the dumps.

    Crickey - how many people ask for help, but get refused?

    You think it will be a waste if time because they will just refuse you again?

    If this nurse is a pain can you ask for a second opinion from a different nurse? Especially if your shrink recommends it?

    What is your anxiety based on? Just wondering if there are any free support groups around. Some churches (more modern ones) have people available to listen and talk through your issues. You can meet people to support you and check up in you.

    anxiety/depression rooted in a troubled mind - thought world a mess.
    doing ok on meds : sertraline and quetapine
    just need some extra professional help like CBT
    ?

    It does sound a bit strange given the time involved. Are you talking to her about any anger or maybe harmful thoughts? If so, maybe she is overreacting thinking you could act in the way you had earlier in life?

    Has she explained why she wants to take the psychoanalytical route to this? If you are no longer the person you were back then and your anxiety/depression is linked to later events, I'm wondering why she can't treat these and leave the past alone.

    Has she said what she hopes to achieve?

    Whilst she may have an obligation to ensure the safety of all clients in a group session, it doesn't sound justified to say you experience "triggers" in CBT given that CBT concentrates on the here & now, not the past. If CBT was a trigger, then surely so would any level of current anxiety/depression and you have said that you have not acted in that way in 24 years.

    I would agree that a second opinion is required here.

    could they not just put you in for 1on1 therapy or like the previous poster put phone therapy?
    I guess it's some sort of obligation with a 'violent' past, it's cruel though to be judging people on events that are so far back when all you're trying to do is better yourself. There must be some way around it.

    I'm waiting on the letter for explanation
    and then see what my options are:
    Both offered above:

    1) get a 2nd opinion
    2) can you do a 1-1 ( don't think they offer cbt on a 1-1 )

    You are doing the right thing.

    Oh, and of the 3 instances you mentioned, the last 2 are very minor and anyone could have done that. How many people have had an altercation in their lives? Punch ups are pretty common on my area for a start!

    The first with the knife where you were sectioned is the one that seems serious and I wonder whether the nurse thinks there is a pattern of violent behaviour and it not looking at the fact you have not done anything to continue that pattern in 24 years? They seem to be suggesting that CBT would bring that back out of you, but I disagree because CBT isn't about the past at all so it leaves me questioning this nurse understanding of CBT.

    I had CBT and whilst it helped to an extent, a lot more work was and still is needed. The therapist recommended continuing with a form of therapy and wrote to my GP. My GP's actual words were "I think the next stage of therapies often rake up more issues than need resolving" and won't do a referral which is ludicrous since he is suggesting those psychologists would make me worse and leave me that way forgetting their responsibilities in their profession. I ignored this reasoning and saw my GP's response as "I'm going to get charged and don't want to be" or "I can't be arsed, bye".

    Its not easy dealing with all this when you are already anxious, but its a sad fact that you have to argue to get what you want with some of these medical professionals!

    That's terrible Terry that your GP would be so mean , hope you find a more sympathetic doctor, because there are many who do take the hippocratic oath seriously.

    seeing my GP tomorrow + increased dosage of quetiapine which psychiatrist has authorised, 100 mg per day ..

    knocked me for 6 this refusal , she could have chosen to encourage me to do CBT and manage any worries she has as regards safety.. its silly and selfish ..
