I'm not a doctor or pharmacist but I can't possibly see how it could or would. It's one of the most widely used OTC pain relievers of the market.

No it can't but your fear of pills could cause a panic attack when taking the panadol.
I know how u feel I panic everytime I take medication doesn't matter what it is but I still panic
Maybe if you were somehow sensitive to it, for example if you were very mildly allergic to something in it. If you were on the autistic spectrum (which many, many people probably are without realising it) you might be able to feel the medication working. Autistic people tend to notice more side effects than would be considered usual. Their bodies can be super-sensitive.
So it's not beyond the realms of possibility that your increased panic could come from somehow sensing the medication starting to work, but this is just a kind of health anxiety.

Panic attacks are not a listed side effect. Anxiety is.
A good test to understand whether it is you or them is does it happen incredibly quickly? A med needs to be digested first and that takes time so unless the side effects indicate an allergic reaction (neither panic attacks or anxiety does), it's likely to be your anxiety about meds.

Check it's not 'fast acting' paracetomol, which may contain (quite a lot of) caffeine.
