I have always held the opinion my panic attacks or anxiety is started with my thoughts but I've read somewhere it starts with physical symptoms.

IMO, it's either or both depending on the person and situation.
The physiological response to a threat, real or perceived, is the "fight or flight response and for the body to produce Adrenalin to help you deal with that threat. That's the rush you feel. So it depends on what your mind perceives as a threat. A niggle of pain can cause that reaction as much as being in a closed space (claustrophobia) etc.

yeah I was thinking back to various ones ive had and sometimes its just some weird stomach sensation or sometimes its just being "trapped" in a waiting room or whatever. I think ive had them about nothing when ive been really exhausted.
my verdict: either.
