I don’t know why foamy urine happens, but when I have it, it tends to be when I haven’t peed in a while, so it’s more concentrated.
Another thing to be wary of, particularly if you’re using a public/office/hospital etc toilet is that the cleaning agents they use in the bowl can linger after flushes, continuing to cause foaming - this happens in the hospital I work at sometimes.
